Home & Community Based Services

Embrace Life Services offers a range of in-home support services, including respite care, habilitation, attendant supervision, and attendant care. We also offer transportation to day programs.

As per our contract with the state of Arizona, we are required to use Electronic Visit Verification (EVV) to comply with federal mandates for paperless documentation and fraud prevention. Embrace Life Services uses MITC for EVV purposes. Please note that live-in caregivers and Direct Care Workers (DCWs) are exempt from using EVV.

Before providers begin delivering services, a Person-Centered Services Plan (PCSP) is required to develop goals and teaching strategies. We maintain close communication with the families and caregivers of the individuals we serve to ensure that the services meet their expectations.

Families should be aware of the type and amount of services authorized by the Division of Developmental Disabilities, as each family's needs differ. Embrace Life Services will ensure you receive the correct authorized services and that they are utilized accurately. If you are unsure, we will work closely with your Support Coordinator to ensure all your service needs are met.

 

Respite & Daily Respite

In caregiving, respite care is a service that gives temporary relief to caregivers/parents who look after someone who cannot care for themselves.

Habilitation Hourly

Part of a certified Direct Care Worker's role is to help a person with a developmental or intellectual disability learn and develop new skills that increase their independence and quality of life. 

Personal care (bathing, dressing, hygiene)

Cooking or meal preparation

Money management and budgeting

Communication and social skills

Learning safety and self-advocacy skills

Attendant Care Supervision/ Attendant care

Certified Direct Care Workers will help members with disabilities, chronic illnesses, or age-related limitations with daily living activities that they can’t safely or independently perform.  Attendant Care provides hands-on assistance rather than skill training (which would fall under habilitation).
The goal is to help a person maintain their daily routine and personal well-being.

Bathing, dressing, grooming, and toileting

Eating or meal preparation

Transferring (moving from bed to chair, etc.)

Medication reminders (non-clinical)

Light housekeeping or laundry

Assistance with mobility or communication

Who qualifies for services through Embrace Life?

In order to qualify for services with Embrace Life Services, members must be supported by the Division of Developmental Disabilities.
