Welcome to Embrace Life Day Program!
Embrace Life provides activities designed to enhance the adult's ability to function in the Day Program in the Hopi and Navajo reservations
The Embrace Life assigned supervisor will develop a teaching strategy plan designed to meet the needs of each member and their families and explore opportunities to improve their daily independent living skills.

Services Inquiry
Intake Process and Timeframe
Intake Process for Members Embrace Life has an electronic intake process. The guardian will need to fill out our intake and pre-orientation forms online to initiate the onboarding process. Upon completion of the forms, your assigned Day Program coordinator will receive a copy and will reach out to you. The coordinator will also contact the assigned support coordinator to obtain the required authorizations prior to services starting. The forms can be accessed at this link:
