Welcome to the Douglas Day Program

Embrace Life provides activities designed to enhance the adult's ability to function as part of a family unit, increasing the family’s ability to care for the child in the home and in community-based settings.

The Embrace Life assigned director will develop a teaching strategy plan designed to meet the needs of each member and their families and explore opportunities to improve their daily independent living skills.

If you are interested in scheduling a tour, please begin by completing the initial non-HIPAA paperwork. Our team will review this information and then contact you to arrange a tour of the day program.
When you have completed the tour and would like to enroll, please fill out the secure online Intake HIPAA form. This will provide our team with the necessary information to process your intake paperwork, including the Person-Centered Service Planning Document and Support Authorization Form.
