Home & Community-Based Services

Embrace Life provides planned and unplanned, routine and non-routine intervals of rest and/or relief to family members or other unpaid persons who reside with and provide ongoing care to members. HCBS includes Respite, Attendant Care, and Habilitation hourly.

DAY PROGRAMS

Our day programs are in most of the big cities in Arizona. Please click to find if there is a day program close to your residence. If preferred, you can also find programs that specifically meet your needs such as support for: Deafness, 1:1 behavior ratios, or medically fragility. Our facilities provide a coordinated program of professional and compassionate services for adults in a community-based group setting. Services are designed to provide social and some health services to adults who need supervised care in a safe place outside their home during the day. Transportation is available if needed.

Transportation

Embrace Life provides safe vehicles for certified providers to transport members for routine and non-routine needs to work programs, academics, vocational rehabilitation services, or to the day program. This is only for non-medical emergencies. In case of any emergency related to urgent medical attention, EMTs will be contacted for further assistance. Please read our program description for further details on how we can assist you with transportation needs.

Day Respite Center

We are currently accepting hourly respite members at our center in Chandler. At the Respite Center, members get together to do basic activities such as coloring, watch movies, crafts, play games, and socialize with supervision from a certified provider. This service provides relief for the caregivers. The center also provides snacks and drinks.

RESIDENTIAL PROGRAMS

This highly individualized program helps a person with developmental disabilities that live in his or her own home with roommates. Direct Care Workers are available 24/7 and provide training for daily living skills, money management, meal preparation, community and safety skills. This program aims to offer the members support so they can have a quality lifestyle in a typical neighborhood setting and can take advantage of what the community has to offer. Read here for more information on some of the best practices we have in our residential programs.

DEAF SERVICES

At Embrace Life, we have a variety of programs that provide full American Sign Language communication accessibility for all our Deaf members. We have trainers, managers, directors and other roles within the agency who can communicate fluently in ASL. The Deaf programs are all staffed by Deaf and hard-of-hearing employees, from the Direct Care Worker level up to management. This means there is not a need to use interpreters to communicate with the members. We have an on-site certified interpreter to assist guardians and the rest of the Embrace Life team with communication as needed.
